It may play a part. Tinnitus is common in general: Mayo Clinic estimates it affects about 15% to 30% of people, and it becomes more common with age. Because menopause arrives in midlife, it can be hard to separate hormonal effects from the normal aging of the ear. What’s clear is that many women report tinnitus starting or getting louder in perimenopause, and it’s one of the symptoms least likely to be raised in a doctor’s office.

Why Does Ringing in the Ears Start in Perimenopause?

The inner ear depends on a steady blood supply and precise nerve signaling. Estrogen influences both blood vessels and the nervous system, and researchers believe the inner ear may be sensitive to its decline, although the research on this is still limited.

Other perimenopause changes can make tinnitus more noticeable. Poor sleep, anxiety, and high stress all turn up the volume on tinnitus, partly because a tired, stressed brain is more alert to internal noise. Does Hormone Therapy Fix It?

Not necessarily, and this is where much of the advice online gets ahead of the evidence. A large study of more than 80,000 women in the Nurses’ Health Study II, published in the journal Menopause in 2017, found that longer use of oral hormone therapy was linked to a higher risk of hearing loss, not a lower one. That study looked at hearing loss rather than tinnitus specifically, but the two often go together. Hormone therapy may be the right choice for other menopause symptoms; just don’t count on it to quiet your ears.

Natural Solutions for Menopause Tinnitus

Get your hearing checked first. Tinnitus is often tied to an underlying cause, most commonly age-related hearing loss, and Mayo Clinic notes that treating that cause often improves it. An audiologist can test your hearing and fit hearing aids if you need them, which helps many people with tinnitus.

Use sound. Background noise such as a fan, white noise, or a sound app makes tinnitus less noticeable, especially at night when it tends to be loudest.

Lower your stress load. Breathing exercises, yoga, and regular movement calm the nervous system, which makes tinnitus less intrusive. Cognitive behavioral therapy designed for tinnitus has good evidence for reducing how much it bothers people.

Protect your sleep. Sleep loss makes tinnitus more noticeable, and tinnitus makes sleep harder. A cool, quiet bedroom with gentle background sound helps break that loop.

Protect your ears. Loud noise damages the inner ear. Wear earplugs at concerts and around loud equipment, and keep headphone volume moderate.

Don’t assume you have to give up coffee. Cutting caffeine is common advice, but a study of more than 65,000 women in the Nurses’ Health Study II found higher caffeine intake was linked to a lower risk of developing tinnitus. If caffeine clearly makes your ringing worse, cut back; otherwise there’s no need to quit.

Tinnitus and emotional overload can feed each other. Constant ringing wears down your patience, and a short fuse makes the ringing harder to tune out. When to See a Doctor About Tinnitus

Make an appointment if tinnitus bothers you, lasts more than a week after a cold, or is affecting your sleep or mood. Tinnitus that pulses in time with your heartbeat, or is only in one ear, also deserves a check.

Get care as soon as possible, per Mayo Clinic’s guidance, if tinnitus comes with sudden hearing loss, dizziness, or numbness or weakness in your face, arm, or leg. Seek emergency care if it starts after an injury, such as a car accident or a hit during sports.

Tinnitus is easy to dismiss and hard to ignore. If the ringing started around the same time as your other menopause symptoms, it’s fair to bring it up, and it’s fair to expect a real answer.
